Persisting gastrointestinal symptoms and post-infectious irritable bowel syndrome following SARS-CoV-2 infection: results from the Arizona CoVHORT

In this study, we aimed to examine the association between gastrointestinal (GI) symptom presence during severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection and the prevalence of GI symptoms and the development of post-infectious irritable bowel syndrome (PI-IBS). We used data from a prospective cohort and logistic regression to examine the association between GI symptom status during confirmed SARS-CoV-2 infection and prevalence of persistent GI symptoms at ≥45 days. We also report the incidence of PI-IBS following SARS-CoV-2 infection. Of the 1475 participants in this study, 33.8% (n = 499) had GI symptoms during acute infection. Cases with acute GI symptoms had an odds of persisting GI symptoms 4 times higher than cases without acute GI symptoms (odds ratio (OR) 4.29, 95% confidence interval (CI) 2.45–7.53); symptoms lasted on average 8 months following infection. Of those with persisting GI symptoms, 67% sought care for their symptoms and incident PI-IBS occurred in 3.0% (n = 15) of participants. Those with acute GI symptoms after SARS-CoV-2 infection are likely to have similar persistent symptoms 45 days and greater. These data indicate that attention to a potential increase in related healthcare needs is warranted.
A lake sediment–based paleoecological reconstruction of late Holocene fire history and vegetation change in Great Basin National Park, Nevada, USA

Analyses of macroscopic charcoal, sediment geochemistry (%C, %N, C/N, δ13C, δ15N), and fossil pollen were conducted on a sediment core recovered from Stella Lake, Nevada, establishing a 2000 year record of fire history and vegetation change for the Great Basin. Charcoal accumulation rates (CHAR) indicate that fire activity, which was minimal from the beginning of the first millennium to AD 750, increased slightly at the onset of the Medieval Climate Anomaly (MCA). Observed changes in catchment vegetation were driven by hydroclimate variability during the early MCA. Two notable increases in CHAR, which occurred during the Little Ice Age (LIA), were identified as major fire events within the catchment. Increased C/N, enriched δ15N, and depleted δ13C values correspond with these events, providing additional evidence for the occurrence of catchment-scale fire events during the late fifteenth and late sixteenth centuries. Shifts in the vegetation community composition and structure accompanied these fires, with Pinus and Picea decreasing in relative abundance and Poaceae increasing in relative abundance following the fire events. During the LIA, the vegetation change and lacustrine geochemical response was most directly influenced by the occurrence of catchment-scale fires, not regional hydroclimate.
High expectancy and early response produce optimal effects in sertraline treatment for post-traumatic stress disorder

Background
Better indicators of prognosis are needed to personalise post-traumatic stress disorder (PTSD) treatments.
AimsWe aimed to evaluate early symptom reduction as a predictor of better outcome and examine predictors of early response.
MethodPatients with PTSD (N = 134) received sertraline or prolonged exposure in a randomised trial. Early response was defined as 20% PTSD symptom reduction by session two and good end-state functioning defined as non-clinical levels of PTSD, depression and anxiety.
ResultsEarly response rates were similar in prolonged exposure and sertraline (40 and 42%), but in sertraline only, early responders were four times more likely to achieve good end-state functioning at post-treatment (Number Needed to Treat = 1.8, 95% CI 1.28–3.00) and final follow-up (Number Needed to Treat = 3.1, 95% CI 1.68–16.71). Better outcome expectations of sertraline also predicted higher likelihood of early response.
ConclusionsHigher expectancy of sertraline coupled with early response may produce a cascade-like effect for optimal conditions for long-term symptom reduction. Therefore, assessing expectations and providing clear treatment rationales may optimise sertraline effects.

Kornerupine and prismatine were introduced independently by Lorenzen in 1884 (but published in 1886 and 1893) and by Sauer in 1886, respectively. Ussing (1889) showed that the two minerals were sufficiently close crystallographically and chemically to be regarded as one species. However, recent analyses of boron using the ion microprobe and crystal structure refinement, indicate that the boron content of one tetrahedral site in kornerupine ranges from 0 to 1. Kornerupine and prismatine, from their respective type localities of Fiskenæsset, Greenland and Waldheim, Germany, are distinct minerals, members of an isomorphic series differing in boron content. For this reason, we re-introduce Sauer’s name prismatine for kornerupines with B > 0.5 atoms per formula unit (p.f.u.) of 22(O,OH,F), and restrict the name kornerupine sensu stricto to kornerupines with B < 0.5 p.f.u. Kornerupine sensu lato is an appropriate group name for kornerupine of unknown boron content. Kornerupine sensu stricto and prismatine from the type localities differ also in Fe2+/Mg ratio, Si − (Mg + Fe2+ + Mn) content, Al content, F content, colour, density, cell parameters, and paragenesis. Both minerals formed under granulite-facies conditions with sapphirine and phlogopite, but kornerupine sensu stricto is associated with anorthite and hornblende or gedrite, whereas prismatine is found with oligoclase (An9–13), sillimanite, garnet, and/or tourmaline. Occurrences at other localities suggest that increasing boron content extends the stability range of prismatine relative to that of kornerupine sensu stricto.

Brandãoite, [BeAl2(PO4)2(OH)2(H2O)4](H2O), is a new Be–Al phosphate mineral from the João Firmino mine, Pomarolli farm region, Divino das Laranjeiras County, Minas Gerais State, Brazil, where it occurs in an albite pocket with other secondary phosphates, including beryllonite, atencioite and zanazziite, in a granitic pegmatite. It occurs as colourless acicular crystals <10 µm wide and <100 µm long that form compact radiating spherical aggregates up to 1.0–1.5 mm across. It is colourless and transparent in single crystals and white in aggregates, has a white streak and a vitreous lustre, is brittle and has conchoidal fracture. Mohs hardness is 6, and the calculated density is 2.353 g/cm3. Brandãoite is biaxial (+), α = 1.544, β = 1.552 and γ = 1.568, all ± 0.002; 2Vobs = 69.7(10)° and 2Vcalc = 71.2°. No pleochroism was observed. Brandãoite is triclinic, space group P$\bar{1}$, a = 6.100(4), b = 8.616(4), c = 10.261(5) Å, α = 93.191(11), β = 95.120(11), γ = 96.863(11)°, V = 532.1(8) Å3 and Z = 2. Chemical analysis of a 4 µm wide needle-shaped crystal by electron microprobe and secondary-ion mass spectrometry gave P2O5 = 28.42, Al2O3 = 20.15, BeO = 4.85, H2O = 21.47 and sum = 74.89 wt.%. The empirical formula, normalised on the basis of 15 anions pfu with (OH) = 2 and (H2O) = 5 apfu (from the crystal structure) is Be0.98Al1.99P2.02H12O15. The crystal structure was solved by direct methods and refined to an R1 index of 7.0%. There are two P sites occupied by P5+, two Al sites occupied by octahedrally coordinated Al3+, and one Be site occupied by tetrahedrally coordinated Be2+. There are fifteen anions, two of which are (OH) groups and five of which are (H2O) groups. The simplified ideal formula is thus [BeAl2(PO4)2(OH)2(H2O)4](H2O) with Z = 2. Beryllium and P tetrahedra share corners to form a four-membered ring. Aluminium octahedra share a common vertex to form an [Al2φ11] dimer, and these dimers are cross-linked by P tetrahedra to form a complex slab of polyhedra parallel to (001). These slabs are cross-linked by BeO2(OH)(H2O) tetrahedra, with interstitial (H2O) groups in channels that extend along [100].
Clinical epidemiology and centre variation in chylothorax rates after cardiac surgery in children: a report from the Pediatric Cardiac Critical Care Consortium

Introduction
Chylothorax after paediatric cardiac surgery incurs significant morbidity; however, a detailed understanding that does not rely on single-centre or administrative data is lacking. We described the present clinical epidemiology of postoperative chylothorax and evaluated variation in rates among centres with a multicentre cohort of patients treated in cardiac ICU.
MethodsThis was a retrospective cohort study using prospectively collected clinical data from the Pediatric Cardiac Critical Care Consortium registry. All postoperative paediatric cardiac surgical patients admitted from October, 2013 to September, 2015 were included. Risk factors for chylothorax and association with outcomes were evaluated using multivariable logistic or linear regression models, as appropriate, accounting for within-centre clustering using generalised estimating equations.
ResultsA total of 4864 surgical hospitalisations from 15 centres were included. Chylothorax occurred in 3.8% (n=185) of hospitalisations. Case-mix-adjusted chylothorax rates varied from 1.5 to 7.6% and were not associated with centre volume. Independent risk factors for chylothorax included age <1 year, non-Caucasian race, single-ventricle physiology, extracardiac anomalies, longer cardiopulmonary bypass time, and thrombosis associated with an upper-extremity central venous line (all p<0.05). Chylothorax was associated with significantly longer duration of postoperative mechanical ventilation, cardiac ICU and hospital length of stay, and higher in-hospital mortality (all p<0.001).
ConclusionsChylothorax after cardiac surgery in children is associated with significant morbidity and mortality. A five-fold variation in chylothorax rates was observed across centres. Future investigations should identify centres most adept at preventing and managing chylothorax and disseminate best practices.

Background
Clinical databases in congenital and paediatric cardiac care provide a foundation for quality improvement, research, policy evaluations and public reporting. Structured audits verifying data integrity allow database users to be confident in these endeavours. We report on the initial audit of the Pediatric Cardiac Critical Care Consortium (PC4) clinical registry.
Materials and methodsParticipants reviewed the entire registry to determine key fields for audit, and defined major and minor discrepancies for the audited variables. In-person audits at the eight initial participating centres were conducted during a 12-month period. The data coordinating centre randomly selected intensive care encounters for review at each site. The audit consisted of source data verification and blinded chart abstraction, comparing findings by the auditors with those entered in the database. We also assessed completeness and timeliness of case submission. Quantitative evaluation of completeness, accuracy, and timeliness of case submission is reported.
ResultsWe audited 434 encounters and 29,476 data fields. The aggregate overall accuracy was 99.1%, and the major discrepancy rate was 0.62%. Across hospitals, the overall accuracy ranged from 96.3 to 99.5%, and the major discrepancy rate ranged from 0.3 to 0.9%; seven of the eight hospitals submitted >90% of cases within 1 month of hospital discharge. There was no evidence for selective case omission.
ConclusionsBased on a rigorous audit process, data submitted to the PC4 clinical registry appear complete, accurate, and timely. The collaborative will maintain ongoing efforts to verify the integrity of the data to promote science that advances quality improvement efforts.

Despite many advances in recent years for patients with critical paediatric and congenital cardiac disease, significant variation in outcomes remains across hospitals. Collaborative quality improvement has enhanced the quality and value of health care across specialties, partly by determining the reasons for variation and targeting strategies to reduce it. Developing an infrastructure for collaborative quality improvement in paediatric cardiac critical care holds promise for developing benchmarks of quality, to reduce preventable mortality and morbidity, optimise the long-term health of patients with critical congenital cardiovascular disease, and reduce unnecessary resource utilisation in the cardiac intensive care unit environment. The Pediatric Cardiac Critical Care Consortium (PC4) has been modelled after successful collaborative quality improvement initiatives, and is positioned to provide the data platform necessary to realise these objectives. We describe the development of PC4 including the philosophical, organisational, and infrastructural components that will facilitate collaborative quality improvement in paediatric cardiac critical care.
